How to get travel health services
Complete and submit the travel assessment form at least 8 weeks before your travel date. One form per traveller. You’ll be added to the wait list for a Tuesday morning travel clinic appointment. A nurse will review your form, assess your needs, and book your appointment or recommend other providers if insufficient time before travel.
