RECEPTIONISTS ASKING FOR FURTHER INFORMATION
When booking an appointment, we may ask you for more information on why you need the appointment. This is because we can make sure that you are directed to the approprate person as we do have a Prescribing Nurse who deals with minor ailments. This free's up some more of the doctors appointment for patients who need to see a doctor. If you do not want to give a reason for the appointment, just explain to the receptionist that it is personal and you would rather not give a reason and they will make an appointment for you with a Doctor.
Many thanks for you co-operation.
Please be aware that if you are going to require travel vaccinations, please make sure that you allow at least 4 to 6 weeks before you date of travel.
Also note that you will need to fill in a risk assessment form before you come for your appointment with the nurse. These forms can be collected from reception or downloaded from this website. You will find them under Forms. The form needs to be handed back to reception at least 3 days prior to your appointment date.
If you require any further information please speak to a receptionist for further details.
